The Study on the Activity of Daily Living Pain in Chronic pain Patients


Abstract
The purposes of this research were to identify Activity of Daily Living(ADL) pain, depression, self-esteem, social adaptation and to examine relationships between ADL pain and depression, self-esteem, social adaptation in patients with chronic
pain.
This a descriptive correlational atudy was used to determine variables related to ADL pain in patient with chronic pain.
A purposive sample of 37 subjects were drawn from rheumatold arthritis patients who were receiving at orthopedic out-patient departments of P university hospital and K hospital in Pusan. Data were collected using a questionnaire and a blood test

The instruments unsed for this study were ; the activity of daily living pain scale developed in this study ; the Zung's Depression Scale ; the Rosenberg's Self-esteem Scale ; the Derogatis' Psychosocial Adjustment to Illness Scale. The collected
data
were analyzed using SPSS PC and were computed frequency, percentage, mean, standard deviation and Peearson correlstion cofficient. The resliability of the instruments were determined by Cronbach's coefficients.

@EN 1. Of 37 subjects, 35(94.8%) were women : the mean age was 40.7; the majority complete high school education ; the average length of prevalence was 57.62 months. Among the activities related to illness, the mean neck stiffness time was 85.78

the mean rest time was 2.11 hrs.. the mean numbers of subcutaneous nodules and swelling joints were 5.27 and 11.00 each, and the mean ESR was 37.05 mm/hr. The mean scores of variables were : ADL pain, 59.12, that indicated moderate pain :
depression,
45.14 : self-esteem, 23.33 : social adaptation, 10.09 on the domain of health care orientation. 14.30 on the domain of vocational environment. 18.39 on the domain of domestic environment, 9.97 on the domain of extended family relationships, 14.27
on the
domain of leisure activity, and 16.81 on the domain of sexual life.
2. There was a statistically significant correlation between ADL pain and rest time among activiles related to illness (r=.3055. P<.05) : it indicates that the more the patients have severe ADL pain, the longer they take rest time.
3. There was no significant correlation between ADL pain and depression (r=.2863. P>.05)
4. There was a negative correlation between ADL, pain and self-esteem (r=-.2934. P<.05) : it indicates that the more the patients have severe ADL pain, the lower they have self-esteem.
5. On the investigation of the relationship between ADL pain and social adaptation, there were negative correlations between ADL pain and the domains of social adaptation ; domestic environment (r=-.3648) : extended family relationship (r-.3236,
P<.05)
: leisure activity (r=-.4185. P<.01). It indicates that the more the patient have sever ADL pain, the worse they have domestic environment, extended family relationship, and leisure activities. Based on the results of the study, it appeared that
rheunmatoid arthritis patients with chronic pain were in pain were in pain during activities of daily living, were depressed, and had low self-esteem and social adaptation level. And the more the patient have severe ADL pain, the longer they have
rest
time, the lower they have self-esteem, and the worse they have domestic environment extended family relationships, and leisure activity. Therefore, developing the nursing intervention for relieving their chronic pain in runmatoid arthritis
patients
will
contribute not only to improving activities of daily living but also to promoting self-esteem and to encouraging social activities.
